"Hills Like White Elephants," like most of Hemingway's work, is lush with symbolism. The hills, along with many other aspects of the story such as the train station, the drinks, and the curtain, serve as a metaphor. Due to Hemingway's subtlety and ambiguity, these symbols can be interpreted subjectively; that is, there are several different interpretations for what the hills themselves can symbolize.
Most specifically, the hills can serve as a metaphor for disagreement. The couple can't seem to understand each other, and when the girl suggests that the hills look like white elephants, they get into a disagreement about the accuracy of the statement. Meanwhile, the couple can't seem to come to an agreement on the topic of abortion. Do the hills actually look like white elephants? Should the girl have the procedure? The couple can't properly communicate, and they can't properly agree.
But why hills? Hills are generic symbols for life's obstacles. Hills jutting out the landscape, in a sense, also resemble the body of a pregnant woman. In either case, the couple cannot agree as to what the hills actually are. As a result, they cannot agree as to what the pregnancy is. Is it a blessing? Is it a curse? Should the baby be birthed, or should an abortion be had? The couple cannot make this decision, and as the story ends, the reader is left without a definite answer.

Do the hills or other elements of the story have symbolic significance?
