There are many different matters to address concerning gender-neutral bathrooms, with the most important real issues often being the least likely to be raised in the impassioned public debates which make restrooms an issue in culture wars.
For small businesses such as coffee shops or restaurants, creating a gender-neutral bathroom can be a simple as putting a sign on a door. Single stall restrooms, which are only used by one person at a time, are inherently gender-neutral. Although traditionally male restrooms have a urinal as well as a toilet, people of all genders can use the facilities. Traditionally female restrooms lack urinals but can still be used by people with male genitalia.
The main practical issues arise in larger public venues. Building codes specify one toilet or equivalent for every 125 men and one for every 65 women. Legal codes would need revision to reflect gender fluidity. Simply creating one stall for every 190 people would be problematic as that does not reflect the pragmatic issue of it taking less time to use a urinal than a toilet, something that affects planning. Single stall facilities or stalls separated by some sort of barriers are more expensive to build than rows of urinals, and thus, costs are important.
On the positive side, redesigning restrooms to be handicapped accessible, single stall, and gender-neutral would benefit everyone. On the negative side, retrofitting existing facilities would be slow and expensive, any changes would require decades.
